Physical Health Related Factors on Suicidal Thought among Community-dwelling Elderly in Korea

Abstract
Objective : The study analyzed the relationship between the physical health problems of the community elderly (65 years old or more) and the suicidal thoughts.

Methods : This study used data from the 2013 Community Health Survey conducted by the Korea Centers for Disease Control and Prevention. In this cross-sectional study, 25,285 men and 36,222 women were included. Physical health problems included the doctor's diagnosis of 10 different chronic diseases, fall experiences within a year, any experience in sick-bed within a month, pain and discomfort. 10 types of chronic diseases were classified into whether the diseases were diagnosed or treated (controlled or worsened), and the number of chronic diseases diagnosed by doctors was estimated for the analyses. Chi-square test was adopted to compare the rate of suicidal thoughts by health problems and complex sample logistics regression to identify the relation with suicidal thoughts. For statistical analyses, IBM SPSS ver. 23 was used and the significance level was defined as 0.05.

Results : Compared with the study subjects without any disease diagnosed by doctors, the risk of suicidal thoughts was 1.30 times (95% CI:1.01-1.69) higher in the subjects with 2 diseases, 1.78 times (95% CI:1.23-2.56) in the subjects with 3 diseases and 2.10 times (95% CI:1.25-3.54) in the subjects with 4 diseases. When it comes to stroke and asthma, the rate of suicidal thoughts in the non-treated subjects (the asthma patients were those that experienced worsening of the disease) was 1.26 times (95% CI:0.99-1.59) and 1.41 times (95% CI:1.06-1.89) higher than that of the non-diagnosed patients respectively. The rate of suicidal thoughts in the subjects that experienced sickness in bed and falls was 1.40 times (95% CI:1.25-1.56) and 1.37 times (95% CI:1.27-1.48) higher than that of the subjects who have no experience. Those who 'slightly' and 'intensely' felt pain and discomfort showed 1.32 times (95% CI:1.20-1.44) and 1.73 times (95% CI:1.50-1.99) higher rate of suicidal thoughts than those who did not felt pain and discomfort.

Conclusions : The study results may be used as reference data for screening high-risk groups of the elderly in the community and developing programs for suicide prevention and intervention.
